Forms may be completed via the following methods:

​1. print, complete, submit a physical copy
2. print, complete, scan as pdf and email to our office
3. complete via our HIPAA compliant e-signature servicer
 
All forms are to be received by our office for processing 24 hours before your initial appointment.

E-Sign Counseling Policy Form

Our counseling policy form must be signed by each client age 18 or older.  Parents/Legal guardians must sign on behalf of clients who are 17 years of age or younger.

E-Sign Adult Intake Form

To be completed by clients age 18 or older for any counseling needs other than
​pre-marital counseling

E-Sign Minor Intake Form

To be completed by a client between the ages of 12 - 17 as a supplement to an adult intake from a parent/guardian.
Minors under the age of 12 must have this form completed by a parent/guardian as a supplement to an adult intake from the parent/guardian.
